I think it's because of host name differences.  It tries to match your
current server hostname and port to what's in your history and passageways
list.  If you added two rooms to your passageways, but they have different
hostnames (such as openverse.org:7000 and datapex4.datapex.com:7000, which
both point to The Living Room), it can't match both.  Even comparing IP
addresses won't work in that particular example (207.0.217.22 versus
206.154.148.4, both ending up at the same machine).

I don't know what to do about it.  Anyone have ideas?

If both the hostname and port match, duplicates are supposed to be
eliminated (only if it's the current server), though now that I look at the
code again, I think that may be broken.  I'll do some testing.

I did, however, just realize that if you have Sort Passageways enabled,
servers in the (unknown) state will appear at the top of your list.  Same
in history.  I'll try and think up a way around that.  Maybe when
converting old history/passageways files to the new format, I'll set their
last visit time to the current time.  This will help, I think, and this
only happens once, when you first run a version of OV tht supports "last
visit" logging.
